Procedure for Loan of Computer and/or Assistive Technology Equipment
- Student Responsibilities:

- Register with the Disability Resource Center (DRC) by following appropriate DRC procedures.
- Provide documentation to verify disability and validate request for services.
- Request use of equipment for a specified/limited amount of time through your disability specialist. Note: some suggested time limits include two days for a test, four days to complete a specific project, etc.
- If approved, contact and meet with Coordinator of Assistive Technology for set up with appropriate computer equipment and instructions on how to use the equipment. Note: The DRC has a limited amount of computer equipment available for approved-student sign-out; loans are made on a "first-come, first-served" and priority basis, as resources allow.
- Fill out paperwork with Coordinator of Assistive Technology, including signing an equipment release form. By signing, student agrees to be responsible for this equipment and its safe return on the date specified.
- Follow policies and procedures for use of equipment as outlined in Assistive Technology Lab Policies and Procedures.
- Maintain proper use and care of equipment.
- Any changes to equipment must be handled by the Coordinator of Assistive Technology. Student is not permitted to load software or make any other significant changes to computer unless given expressed permission by the Coordinator (in writing).
- If you notice a malfunction in the operation of the equipment, immediately return equipment to DRC for repair and/or replacement.
- Bring the equipment in to DRC by deadline given on sign-out paperwork. After the equipment is examined and found to be functioning properly, the student may request to re-check out equipment, as needed.
- Disability Resource Center Responsibilities:

- Disability specialist will present student's request, along with documentation, to other professional staff to determine if request is an appropriate accommodation. Check-out time frame is determined by DRC in staffing.
- If approved, disability specialist will notify Coordinator of Assistive Technology of request and check-out time frame.
- Coordinator of Assistive Technology will handle set up for student with appropriate equipment and give instructions on how to use the equipment.
- Coordinator of Assistive Technology will maintain paperwork on sign-out of computer and/or assistive technology equipment.
- When equipment is brought back in, the DRC will make sure the equipment is functioning properly, handle check-in paperwork, and will re-check-out equipment as appropriate.
Note: Any equipment, software and/or materials loaned by the DRC to students, faculty or other UGA departments, remains the property of the DRC and may not be copied or relocated for personal use. The DRC has the right to recall at any time, any piece of the DRC equipment which is checked out. the DRC will FLAG the student's record if student fails to return equipment.
